Perpetua da Silva applied to emigrate to the Sandwich Islands on May 29, 1884 in Funchal, Madeira. The official file appears to be No. 484. She is recorded on the passport with her parents, Francisco da Silva and Maria Roza, natives of the parish of Arco da Calheta and residents of Pinheiro, and her siblings Maria, João, Francisco, and Ludovina. An extraction of her baptism record is included in the application file and attests that on the 8 June 1879 in the parish of São Brás of Arco da Calheta was baptized Perpetua, legitimate daughter of Francisco da Silva das Fontes and Roza de Jesus, natives of this parish and residents of the village of Pinheiro. Granddaughter on her father's side of Francisco da Silva das Fontes and Isabel de Jesus, and on her mother's side of Antonio Fernandes and Anna de Jesus. Godparents were Fernando de Ponte Lyra and Perpetua de Jesus. Extracted from the original 17 May 1884 by vicar Ricardo Jose de Franca Doria.

Extract translated from Portuguese: On this eigth day of June of the year one thousand eight hundred and seventy nine in this Church of São Braz of Arco da Calheta, County of Calheta, Diocese of Funchal, I have solemnly baptized an individual of the female gender, who I have christened as Perpetua, who was born in this parish at six in the afternoon on the twenty third day of May of this same year. Legitimate daughter (first of this name) of Francisco da Silva das Fontes, farmer, and Roza de Jesus, housewife, born in this village of Arco da Calheta where they were married and residents of Pinheiro. Granddaughter on her father's side of Antonio da Silva das Fontes and Isabel de Jesus, and on her mother's side of Antonio Fernandes and Anna de Jesus.
